Five reels in a 3-2-3-2-3 layout give 108 ways by default, and a way pays left to right on adjacent reels. Patient-1 is the top symbol at 5x the bet for five, the organ symbols pay between 0.10x and 1.25x. Every base spin puts Fire Frames on 1 to 13 positions, and a symbol landing in a frame splits into two.

Under reels 2 and 4 sit Enhancer Cells that open after 4 and 6 Fire Frames and reveal a Patient, a Dead Patient, xWays, xSplit, a Wild or an xNudge Wild. xWays turns into three symbols of one kind and stretches the reel, while xSplit splits one symbol on each reel into two and becomes two Wilds itself. Two Dead Patient symbols reveal a Patient carrying a multiplier from x5 to x9999 that counts as that many copies on its reel.

A Spider on reel 2 or 4 triggers Mental Transform into xWays, xSplit, a Dead Patient, a Wild or a Patient, and an xNudge Wild adds +1 to its multiplier for every step it nudges. Three Scorpions, a symbol limited to reels one, three and five, start 8 Autopsy Freespins, +1 for each Scorpion inside a Fire Frame, and during the feature up to five new Fire Frames per spin stay until the end.

A Spider during Autopsy upgrades the rest to Lobotomy Freespins (+1 spin), where the Spider sticks, Mental Transform runs every spin and Dead multipliers are collected and handed to matching Patients. Two Spiders, or a new one during Lobotomy, lead to Mental Freespins, where that count no longer runs down. Splits carry the base game

The best round was a natural feature worth 280.4x at round 9533; remove it and the remaining 9,999 spins come to 80.92%. Nothing near the 66,666x cap turned up; the rules put that top prize at 1 in 18,000,000 simulated rounds.

Natural features came 49 times, once per 204.1 spins, and their free spins averaged 67.33x the stake with a median of 56.9x. Only four of them reached Lobotomy, three by upgrade and one straight from the base game, and none got to Mental Freespins. Those four averaged 91.36x against 65.19x for the 45 that stayed in Autopsy.
